- Single Individual = $30.26 per month
- Single + 1 Dependent (Spouse or Child) = $63.55 per month
- Single + 2 or more Dependents = $94.39 per month
How Do I Enroll?
- Review the below document links and complete the "Application and AutoPay Authorization for Payment" forms
- Scan and email completed documents with "VOID" check copy to john.harder@schellerbradford.com
- Member must email forms by the 10th of the month for dental insurance coverage to begin the next 1st of the month
